READ BEFORE SIGNING!

In applying to the Jupiter Pro Am Fall Classic surfing competitions, I promise to inspect the contest site, and assure myself that the area is safe for surfing, and further agree that I will not surf in the contest, unless I am satisfied that the area and conditions are safe for surfing purposes. In exchange for the Jupiter Pro Am Fall Classic accepting my entry form, I voluntarily agree to assume all risks incident to the sport of surfing. I fully understand and comprehend the dangers of surfing created by the sharp edges and fins along with the natural buoyancy of a surfboard when they are acted upon the powerful forces of the ocean and ocean waves. With full acknowledgement, comprehension and understanding of these dangers, I voluntarily accept and assume all risks involved in the spirit of competitive surfing. I will further read and comply with all the Jupiter Pro Am Fall Classic Surfing rules and regulations, and do agree that, noting existing weather conditions, I voluntarily assume all risks arising from conditions related to use of the contest site by myself and others. I give the Jupiter Pro Am Fall Classic permission to arrange to telecast, show, or print and publish any and/or all pictures taken of me (movie, video, or still) in conjunction with said event. In consideration of your acceptance of my entry, I intend to be legally bound, hereby, for myself, my heirs, executors and administrators, hold harmless and release and forever discharge the Jupiter Pro Am Fall Classic Surfing Competition, Jupiter, county of Palm Beach state of Florida, any and all sponsors, co-sponsors, named or not, members, agents and any and all officials connected with this competition, from all liabilities for injuries and damages whatsoever, arising from my presence or participation in the above described event and do hereby grant the sponsors such release as described herein. I acknowledge that I have read and understand all of the above. My signature below indicates I will abide by all competition rules; I will not enter the competition area unless competing and I will not demonstrate any unsportsmanlike conduct. Any violation of the above will be cause for disqualification.

FOR CONTESTANTS UNDER 18 YEARS OF AGE: I HEREBY CERTIFY that I am the parent or guardian of the surfer named above, and do hereby give my permission without reservation and with full understanding and comprehension of the foregoing agreement to hold the aforementioned organization and governmental entities harmless any and all liabilities including any such injuries which are alleged to have occurred as a result of negligent failure of Jupiter Pro Am Fall Classic officials, judges, and directors to properly supervise contestants in the course of competition.
